Need scan of an EFP Nameplate

I'm building a frankenrig out of an old EFP, and I'm trying to restore the nameplate on the back of the sled, which has seen better days. Is there any way a fellow EFP owner could scan their nameplate for me? I'm including a picture of the one I have, for reference.
